Appeal of Financial Aid Suspension
Form used to appeal student's Financial Aid Suspension. Attach additional documentation, if needed.
0809 - Dependent Verification Worksheet
If you are a dependent student and have been selected by the Department of Education for verification, this is a required form to complete your file. Be sure to obtain appropriate signatures or the form will be returned to you unprocessed.
0809 - FAFSA Drug Worksheet
Department of Education Drug Worksheet used to answer the drug-related conviction question on a submitted FAFSA.
Graduate/Degree Verification Form
Form used to verify information submitted on the FAFSA that a student has a Bachelors/Master Degree.
0809 - Independent Verification Worksheet
If you are an independent student and have been selected by the Department of Education for verification, this is a required form to complete your file. Be sure to sign the form or it will be returned to you unprocessed.
0809 - ISIR Signature Page
If you did not sign your FAFSA, use this form and return to STC in order to allow a correction to be made to your application.
Selective Service Verification Form
Form used to verify student's Selective Service Status. Attach additional documentation, if needed.
0809 - STC Cert, Auth & Agree Stmt
Form required to complete student's financial aid file for Federal and State Aid. Please sign, date, and return to the Office of Financial Aid.
Request for Transcript of Tax Return
Form used to request tax return transcripts, tax account transcripts, W-2 information, 1099 information, verification of non-filing, and a record of account. If you are selected for verification and do not have a copy of your Tax Return or did not file for the current year, you can use this form to request your IRS Transcript.
0809 - No Income Verification Form
Form used to verify income of students who are not required to file a Federal Tax Return when applying for Federal Aid.
Please use "Blue" or "Black" Ink Only when filling out all Applications and Financial Aid Paperwork!
Note: You cannot complete any of these forms online. You must print them, fill-in the required information, and return them to the Office of Financial Aid. Please make sure you sign all forms and include your Student ID/Social Security Number.
